Welfare Note #18
Another offering in a series of "Welfare Notes" that has been created by the Welfare Committee which are intended to ensure that all members of the Faculty Association are educated, informed, or updated in terms of welfare-related concerns and their rights contained within the Contractual Agreement.
Evaluation Procedures and Forms
There were several changes made to the evaluation procedure and/or evaluation forms in the 2007-2011 Contractual Agreement between CODFA and the Board.
Assessment and Evaluation of Tenured Faculty (Section D 9.1 and forms in Appendix A)
The Evaluation Cycle consists of the same three phases—Planning, Formal Evaluation, and Update. The Faculty Member still prepares a Phase I Planning Document, meets with the Evaluator, and plans for the Formal Evaluation.
- The Record of Agreement prepared by the Evaluator has been done away with. Instead the Phase I Planning Document prepared by the Faculty Member is the document that is signed by all parties and sent to Human Resources
- There was a slight change to the weeks in which the Student Rating Questionnaires are administered and the questionnaire has a student comment section. The Phase II Self Evaluation Form was unchanged
- The additional category of "needs improvement" was added to "satisfactory" and "unsatisfactory" in The Formal Evaluation Report
- The Update Conference form is now optional for the Faculty Member. It can be filled out to update any achievements or goals or it can be skipped. The Evaluator fills out and submits the unchanged Update Conference Report
- Bottom Line—The Evaluator and Faculty Member each have one less required form to fill out
Assessment and Evaluation of Non-Tenured Faculty (Section D 9.2 and forms in Appendix A)
There are no changes to the process. The Record of Agreement form for Non-Tenured Faculty is prepared by the Evaluator. The Classroom Observation Form had language cleaned up and clarified.
Assessment and Evaluation of Tenured and Non-Tenured Counseling Faculty (Sections D 9.3 and D 9.4 and forms in Appendix A)
A Phase I Planning Document—Counseling Faculty Member form and a Phase I Record of Agreement Non-Tenured Counseling Faculty form were added to make the forms for counseling faculty parallel to those for two-semester faculty.
Assessment and Evaluation of Tenured and Non-Tenured Library Faculty (Section D 9.5 and forms in Appendix A)
There are no changes to the process. Contractual language was changed in one place to reference the correct form.
